If you are interested in attending the TAKE CHARGE™ Program, please note the following requirements before applying:
1. Verify that you are able to travel on June 20-22, 2014. You must be present for the duration of the entire program.
2. You and your support partner are responsible for your own travel expenses to and from the Oakland City Center Marriott in Oakland, CA, the $25 daily parking fee and any additional hotel incidentals (ie, room service, wifi internet, in-room movies, etc).
3. You must complete the below program application online in its entirety.
4. You must print, complete and send back (via mail, fax or email) the $75 Application Payment Form in order for your application to be complete. You may click here to download this form.
This is a very popular program, and it has limited availability. Please do not delay in the application process; this program will fill quickly.
NOTE: Applying to this program does NOT mean you are automatically accepted into the program or into future programs. Admittance is first come, first serve once your application has been reviewed and accepted.
*All applicants will be notified if they are accepted into the program within two weeks of submitting their completed application and payment form to Can Do MS.
