How to Apply

CAN DO Programs are limited to 24 participants and their support partners. These programs fill quickly. Please apply by the application deadline stated below.

Each applicant will need to provide Can Do MS with the following:

1. A completed and signed application by the deadlines stated below
–Call Can Do MS at 1-800-367-3101 to request a program application

APPLICATION DEADLINES

• May 2- 5, 2012 – Vail, CO – Vail Marriott Mountain Resort & Spa
  - One Night Free Lodging Deadline: Apply by Monday, February 20, 2012 and receive one night of free lodging at the Vail Marriott Mountain Resort & Spa.
  - Application Deadline: Monday, March 19, 2012
• November 7 – 10, 2012 – Vail, CO – Vail Marriott Mountain Resort & Spa
  -One Night Free Lodging Deadline: Apply by Monday, August 26, 2012 and receive one night of free lodging at the Vail Marriott Mountain Resort & Spa.
  - Application Deadline: Monday, September 23, 2012

2. A signed copy of the Cancellation Policy

3. Medical release from either your primary care physician or neurologist: One copy of the Request for Medical Information letter and form has been provided in the application packet for your convenience.

4. A letter from your psychotherapist, if currently receiving treatment, recommending your participation in the program
